Code of Conduct Documents for Funds
The following documents have been developed by Private Healthcare Australia and the Members Health Fund Alliance to assist health funds in the implementation of the Private Health Insurance Code of Conduct.
Self-Audit and Certifications
Health funds who intend to be signatories to the PHI Code of Conduct must complete a detailed self-audit and be approved to participate in the Code processes by the Code of Conduct Compliance Committee (Committee).
A signatory health fund that has been admitted to participate in the Code and that has satisfied the Committee that it continues to be compliant with the Code requirements, is able (once authorised by the Committee) to utilise the logo and style guide.
The self-audit certification form document provided above comes with writeable data fields for completing and digitally signing the self-audit. The accompanying instructions (form completion guidance) are as follows:
- You need to save a local copy of the fillable PDF form document first, in order to insert the required authorised digitised signatures (handwritten signature image) throughout the document at the appropriate fields.
- Name the saved file appropriately to include your signatory health fund’s name.
- Open the saved PDF form document in either Adobe Acrobat Reader (free version) or Adobe Acrobat Standard/Pro.
- To enter responses into a field, click once and then simply start typing.
- In the fields requiring ‘tick’ responses, ticks will be entered by a simple click, or deleted via clicking again in that field.
- Include the required authorised digitised signatures (handwritten signature image) at the appropriate fields.
- The form document includes additional space for answers that appear in a section Supporting Document Summary (pg 33).
- Once completed, save the changes.
- The final completed form document needs to be uploaded by the signatory health fund to their own secure section on the Self Audit Compliance Program Portal.
PHI Code of Conduct Logo Usage
The Private Health Insurance Code of Conduct logo has been developed for use by health funds once they have submitted the relevant Self-Audit and have been authorised by the Committee to use the logo.
Following authorisation by the Committee, health funds must incorporate the PHI Code of Conduct logo into marketing material and other documentation at the first available opportunity and in line with the logo style guide.
